  • Documents proving your condition or disability. Talk to your GP for what you’ll need.  

Who can give evidence of your condition? 

As you would expect, the NDIS will need evidence of your condition, and as such your treating professional will need to complete their part of the application. Your professional may be: 

  • A doctor
  • A specialist

  • An allied health service provider

Your treating professional must also: 

  • Work with you for more than 6 months 

  • Is the most appropriate type of professional for your condition

  • Is qualified and registered in their area with the Australian Health Practitioner Regulation Agency, or relevant professional authority.

How do you apply for the NDIS? 

The way that you apply for the NDIS depends on how old you are, and no matter how old you are, you have a few different ways you can apply: 

If you’re over seven years old

  1. Your local area coordinator or local NDIS office can help guide you through the process. For all your help, find the location near you

  2. You can call the NDIS at 1800 800 110

  3. You can fill out the NDIS application form online

If you’re younger than seven years old 

If your child is younger than seven years old, after talking to your health or education professional, you should talk to an NDIS early childhood partner. An early childhood partner can point you in the right direction on how to get NDIS support. Usually you will:  

  • Need to give the NDIS the information needed to prove your identity

  • Provide information so the NDIS can decide if you’re eligible

  • Sign or certify the NDIS application
